Service Description
Not sure which laboratory test would provide the most useful information for your concerns? This individualized consultation is designed to help you explore testing options based on your health history, current symptoms, lifestyle, wellness goals and any previous laboratory results. Depending on your needs, personalized testing options may include DUTCH hormone testing, GI-MAP digestive testing, NutrEval nutritional and metabolic testing, or other functional wellness labs. We’ll discuss what each test evaluates, why it may or may not be appropriate and how the testing process works. You may choose to meet by secure video or phone. Following your consultation, you’ll receive your personalized testing recommendations and ordering instructions by email. Best for: Individuals who want more specialized testing or need guidance determining which lab options best match their concerns. What’s included: - Pre-session review of your intake information - Private video or phone consultation - Review of relevant symptoms, history and previous testing - Education about appropriate laboratory options - Personalized testing recommendations delivered by email - Ordering and collection instructions Please note: Laboratory costs are separate from the consultation fee and vary according to the testing selected. Results interpretation and development of a wellness roadmap are booked separately unless specifically included with your testing package.
Cancellation Policy
By booking this service, you acknowledge and agree to the following: Scope of Service: This consultation provides wellness education and guidance in selecting laboratory testing based on your intake information, health history, concerns, previous results and wellness goals. Positive Shift Wellness does not diagnose, treat, cure or prevent medical conditions, and this service does not replace care from a licensed medical provider. What Is Included: Your booking includes review of submitted intake information, one 60-minute video or phone consultation, and personalized testing recommendations and ordering instructions delivered by email. Not Included: Laboratory fees, specimen collection, shipping, results interpretation, a written wellness roadmap, supplements and follow-up support are not included unless specifically stated. Results interpretation may be booked separately after testing is complete. Payment: The $150 consultation fee is due in full when booking. Laboratory fees are purchased separately and vary according to the tests selected. Recommendations do not obligate you to purchase testing. Cancellations and Rescheduling: Please provide at least 24 hours’ notice to cancel or reschedule. Cancellations made after intake review has begun may be subject to a $75 preparation fee. Appointments canceled with less than 24 hours’ notice and missed appointments are nonrefundable. Late arrival does not extend the scheduled appointment. Laboratory Services: Testing is processed by independent laboratories. Positive Shift Wellness does not control laboratory pricing, availability, collection requirements, shipping, processing times, result accuracy or refund policies. Certain tests may not be available in every state. You are responsible for following all collection and shipping instructions. Medical Care: Discuss significant symptoms, abnormal findings and changes to medications or treatment with your licensed healthcare provider. Never discontinue medication or delay medical care based on this consultation. This service is not appropriate for emergencies. No Guaranteed Outcome: Laboratory testing may provide helpful educational information, but no particular finding, explanation or wellness outcome is guaranteed. HSA/FSA: Some consultation or testing expenses may qualify for HSA/FSA use depending on your plan and applicable requirements. Positive Shift Wellness does not guarantee eligibility or reimbursement.
